Is your Child the lucky recipient of one of our Birthday Invitations or part of a school or group coming to the gym?

If so, to be able to participate in activities we require that the responsible parent or guardian agrees to the conditions of participation via this consent form.

What should I wear: Tight fitting clothing, Bike Pants, Singlet, Bummers etc.

What not to wear:

- Jewellery, necklaces, rings, watches or belts as these could cause harm to the kids, coaches and equipment.
- Dresses or Jeans.

Terms and Conditions

I acknowledge that I am aware there is an inherent risk of injury or ill-health resulting from the use of North West Gymnastics facilities and services, and from participation in exercise generally.

I therefore undertake to utilize these facilities and services only at my own risk and I hereby waive on behalf of myself, my heirs and executors hereafter liability against North West Gymnastics for any injury, illness or adverse change in my medical condition or state of health (whether permanent or temporary) arising directly or indirectly from my use of the North West Gymnastics facilities or services provided, whether supervised or not by North West Gymnastics staff.

I give permission for my child to receive medical/ambulance assistance in case of injury or emergency and agree to pay such costs incurred.

The information provided on this form is complete and correct to the best of my knowledge and I undertake to advise the Club promptly of any changes that may occur.
